Incident Form


The Incident form continues to have functionality added to it for purposes of satisfying additional law enforcement reporting requirements and data sharing initiatives. An Incident form report may be used for in-house data collection as well as for satisfying numerous law enforcement reporting requirements.

    • Other: More agency notifications to come. Currently under consideration are:

  • Wisconsin Department of Corrections (DOC): Department of Corrections notifications for law enforcement contacts and searches of subjects on extended supervision (probation/parole) and/or registered sex offenders. (Wisconsin Legislature: 2013 Wisconsin Act 79)

    • Child Protective Services - TBD

  • Wisconsin Department of Justice (DOJ) is set to interface with the Incident form in the future for purposes of reporting suspected human trafficking, and for Suspicious Activity Reporting (SAR).

    • Agencies are currently encouraged to participate in the Nationwide SAR Initiative by forwarding suspicious activity reports to their designated fusion center or appropriate federal office. The Southeast Threat Analysis Center (STAC) is the designated fusion center serving the eight counties of Milwaukee, Racine, Washington, Waukesha, Ozaukee, Kenosha, Walworth, and Jefferson, while the Wisconsin Statewide Intelligence Center (WSIC) is the designated fusion center for all other Wisconsin counties. Suspicious activity reports can be submitted to the STAC by calling 1-877-WIWATCH (1-877-949-2824) or emailing STAC@milwaukee.gov. Suspicious activity reports may be shared with WSIC by using WILENET, navigating to the WI Fusion Center webpage (About the Wisconsin Fusion Center | WISCONSIN STATEWIDE INTELLIGENCE CENTER (WSIC) ), emailing wsic@doj.state.wi.us, or calling 1-888-DCI-WSIC.

  • Campus Security Authority and Local Police: Cleary Act reporting. (2014-CFR Part 668) (Understanding-Clery-Statistics) (The Clery Act)

The Incident form is in phase three of the incident project. It is a basic incident form with fields needed from Law enforcement agencies, Department of Justice, District Attorneys DAIT Protect software, Department of Corrections, HIDTA and Dept. of Safety and Professional Services.

  1. In Phase one agencies can fill out internal reports and eReferral to the DA.

  2. Phase 2 was transmission to the DOJ for federal IBRS related requirements.

Document Group

Each Incident form contains one Document group. The fields within the group are shown in the figure below. The following are the dispositions that could be set for the form

  • Unfounded

  • Offender Death

  • Declined

  • Other Jurisdiction

  • Victim Refused

  • Juvenile

  • Referred to DA

Incident Report Type

This currently has 4 options only 2 can be chosen together.

  • Agency Internal report Only – can only be chosen by self

  • No incident for DOJ for the month—can only be chosen by self

  • DA eReferral

  • WIBRS Report

  • Overdose

Reporting Month

This is only used with the no incident for DOJ for the month report

Reporting Year

This is only used with the no incident for DOJ for the month report

Agency Report Type

This is an agency dropdown list to be populated by a TraCS administrator.

Transmission To

This is all the locations that the form will transmitted during the next transmit.

  • EREFERRAL TO DAIT – Live need setup

  • IBR TO DOJ -- Live need certification

  • PRESCRIPTION DRUG MONITORING PROGRAM – Projected March 2025

  • OVERDOSE DETECTION MAPPING APPLICATION PROGRAM – Live

Initial Validation Date

First time the form is validated successfully.

Date Cleared

This gets entered when the form is marked. Which is accomplished by hitting the corresponding buttons in the Incident Ribbon.

Transmission History Group

This section is only found only if the Incident has been transmitted. It is a history of all the transmissions to the state that have taken place and is not editable.

Transmission To

This is all the locations transmitted to during the last transmission attempt.

Summary Group

This section contains general information about the accident. Every Incident report contains one summary group. The green highlighted fields are form mechanic fields and should always be entered for the form to work properly.

 

eReferral

This demines whether or not the form will Transmit to DA’s Protect system. It also triggers rules closely following a NTC and ELCI citation validation rules

eReferral Routing Override

Only fill out this field if you are sending the referral to a DA that is not your default DA’s office.

eReferral Retransmit

This field is only enabled if the forms has been transmitted to the Protect system already. This will trigger a retransmit to the Protect system. A retransmit should only be considered when big changes are made, for example finding another victim or modifying charges. Not for small items like a change in address.

Is Date/Time of Incident Known fields

These fields enable and disable the upper range fields.

Total fields

They are an easy way of getting the groups to open.

Overdose Type

This is a HIDTA field and will eventually trigger a transition to HIDTA when we get the transmission up and running in a few years.

Prescription Alert Type

This is a Prescription Drug Monitoring Program field and will eventually trigger a transition to Prescription Drug Monitoring Program when we get the transmission up and running in a few years.

Transmit to DOC

This is to trigger a transmission to the Department of corrections and will be yes if any subject is on probation, parole or is a registered sex offender. If it triggers, consider emailing the department of corrections a copy of the report.

Transmit to DOJ

This is to trigger a transmission to the Department of Justice and will be yes if any for all violations that aren’t set to runaway or not reportable.

Delete from DOJ

This is to trigger a transmission to the Department of Justice.

Date Deleted

Date the Delete was set

Location Group

Offense Group

Each Incident form can contain 1-99 Offense group. The fields within the group are shown in the figure below.

Prescription Status

This is a Prescription Drug Monitoring Program field and will eventually trigger a transition to Prescription Drug Monitoring Program when we get the transmission up and running in a few years. This field will eventually repopulate based on the statute.

Offense

This field is a required field and will trigger multiple rules and behaviors in the form. For example the following fields will adjust according to the offense type

  • Attempted Completed

  • Premises entered

  • Method of entry

  • Suspected of using

  • Bias motivation

  • Criminal activity

  • weapon

Person/Property/Society

This field determines the type of offense and where in the multiple drop down lists it can exist.

Injury Offense

This field will trigger additional fields in a victim is this offense is against the victim.

Multi Off Codes

This field is triggered off the statute description field and will display multiple offense choices

Dup Offense

If a duplicate offense is chosen.

Subject Group

Each Incident contains 1-99 subject group. The fields within the group are shown in the figure below. If a company is cited, the Defendant Type field must be Non-Individual which will disable the name fields. While in the Last Name field click the External Search button (If configured) to populate the databar then highlight individual.

 

Individual Type

Determines how the fields are enabled and disabled.

  • Individual

  • Non-Individual

  • Unknown-Will not work for a DA eReferral

Associated Offenses

Links to the offenses on the form.

On Probation or Parole

This is for the Department of Corrections anytime an individual is involved in something and is on probation or parole the department of corrections would like a copy of the incident.

Vehicle Involved

This field opens up a vehicle subgroup. This field is at the request of the department of Corrections. They like to keep track of individuals on probation or Parole and their vehicles along with whether or not they are renting vehicles. As of March 2024 this field is available for any type of incident.

Registered Sex Offender

This is for the Department of Corrections anytime an individual is involved in something and is a registered Sex Offender the department of corrections would like a copy of the incident.

Arrested or Booked

This will open the arrested/booked subgroup and have fields required for the Department of Justice and the DA’s office

Prescription Violation

This is a Prescription Drug Monitoring Program field and will eventually trigger a transition to Prescription Drug Monitoring Program when we get the transmission up and running in a few years. This will enable/disable the prescription subgroup field.

Overdose

This is a HIDTA field and will eventually trigger a transition to HIDTA when we get the transmission up and running in a few years. This will enable/disable the prescription subgroup field.
