WI DSPS/DOJ/DHS: Overdose Reporting
Note: The Suspected Overdose Report is currently in the planning and outreach stages. Use of this form is not active at this time. There will be widespread communication from the Wisconsin Department of Justice and other partners when the form is ready for full utilization. In 2024 transmission of overdose incidents was added to the Incident form. Suspected overdose reports will be sent to the following agencies:
Wisconsin Department of Safety and Professional Services (DSPS) to satisfy the law enforcement mandated reporting of suspected overdoses, diverted substances, and controlled substance theft to the Prescription Drug Monitoring Program (PDMP) according to state statute §961.37;
Wisconsin Department of Justice to support coordinated law enforcement overdose intelligence efforts across the state; and
Wisconsin Department of Health Services to include law enforcement encountered suspected overdoses anonymously within the Wisconsin Suspected Overdose Alerts for Rapid Response (WiSOARR) system for overdose alert monitoring and prevention efforts.
The minimum required information for each of these efforts will be sent from submitted suspected overdose reports. The purpose of this form is to fulfill law enforcement reporting requirements while simultaneously reducing reporting burdens to additional agencies and supporting statewide overdose prevention and response efforts.
