How To Complete a CMV(NON-MCSAP)Inspection Report

Inspection Report Type: CMV Inspection (Non-MCSAP)


The inspection form is to be used for all traffic stops involving commercial motor vehicles. This includes any vehicle in commerce, private or for hire, with a gross vehicle weight rating, gross combination vehicle weight rating, gross weight or gross combination of 10,001lbs or more whichever is highest. The form may also be used to document stops involving combination vehicles not in commerce. This guide is intended to help complete the inspection form giving instruction and information on some of the more uncommon fields.

Carrier information may be obtained from the bill of lading, Lease Agreement, Power unit markings, Registration cab card, log book or a combination of them all.

 

 

 

US DOT # Field - Enter the USDOT number of the carrier whose operating authority is being used to haul the load. US DOT numbers can be entered and searched using the web search button as long as the computer being used has an internet connection.

This button looks up the carrier name associated with the USDOT number entered and then fills in the carrier information from the federal database.

LC/PC # Field - For Intrastate carriers enter the common carrier authority (LC) or passenger carrier authority (PC) if applicable. The authority numbers may be found on the Wisconsin cab card.

The carrier contact information fields do not need to be completed for a roadside stop.

Commerce Type Field – Select whether the carrier is Interstate or Intrastate: (not based on the load that day but overall operation)

A. Interstate Carrier: Vehicle to be inspected has a GVWR of 10,001lbs or more, single vehicle or combination, does trade, traffic, or transportation is between a place in a state and a place outside of such state (including a place outside of the United States); between two places in a state through another state or a place outside of the United States; or between two places in a state as part of trade, traffic, or transportation originating or terminating outside the state or the United States.

B. Intrastate Carrier: Vehicle to be inspected has a GVWR of 10,001lbs or more, single vehicle or combination, and does trade, traffic, or transportation exclusively in business’s domicile state.

D. Not in Commerce/Government: Vehicle to be inspected has a GVWR of 10,001lbs or more, single vehicle or combination, and is owned and operated by a government entity such as a highway department or a city transit department.

C. Not in Commerce/Other Truck: Vehicle to be inspected has a GVWR of 10,001lbs or more, single vehicle or combination, and is owned and operated by a private individual not associated with any business.

E. Other Operation /not specified: An example of this could be a vehicle or vehicle combination with a GVWR or GCWR under 10,000lbs or in the case of a crash, a hit and run vehicle where there is no information.

Complete the driver group in similar fashion to other TraCS forms.

Origin and Destination City and State fields: Enter the original origin and final destination of the load being hauled.

Shipping Paper#: Enter the bill of lading number or pro number corresponding to the load being hauled.

Cargo field: Use the dropdown list to populate the cargo being hauled. If the cargo is not listed select other and enter the commodity being hauled.

Cargo Breakdown field: Does not need to be completed

Shipper Name: Enter the name of the individual or company paying for the load to be shipped.

Brakes Inspected? Field: In most cases unless specifically trained to inspect brakes the field should be marked “No”.

Certified to Inspect Brakes?:The brake inspection group should not be completed unless the individual completing the report is certified to complete a North American Standard Level I inspection having received Federal Motor Carrier Safety Administration training sponsored by the Wisconsin State Patrol.

 

If a Brake Inspection is Completed:

If you are certified to inspect brakes and a brake inspection was completed on a vehicle which would not qualify for a inspection using the Federal Motor Carrier Safety Administration inspection software ASPEN complete the following steps. Select “Yes” in both the “Brake Inspected” and “Certified to Inspect Brakes?” fields. The “Chamber” field will then open under Axle 1.

 

When the user selects the “Axle 1” field an area will open above which activates a “Brake” botton.

 

Select brake button and a Brake Measurement window will open allowing the user to complete the brake section for each axle up to 20 axles.

 

 

 

1. Select the Chamber (Brake) type for the axle first.

2. Select the chamber size next if applicable.

3. Select the measurement taken for the chamber on the left and right side of the axle.

4. Select the Next button to move to the next axle and enter its brake information.

5. When all axles have been entered select the OK button. The window will close and all the entered information will popuplate on the form.

Statute Rule Number Field: Infromation entered and searched in the same manner as the TraCS warning and citation forms.

Remarks/Explanation Field: Additional information can be entered here regarding the violation.

RDI? Field: Field defaults to “No”and refere to and item Repaired During Inspection. Select “Yes” if items are repaired during the inspection.

Citation Given? Field: Field is defaulted to “No”. Select “Yes” if a citation is to be issued for the violation selected in the Statue Rule Number field. Selecting “Yes” will open the “Issue To?”, “UCT Number “, and “Chk” fields.

If “Yes” selected for the Citation given field:

Go to the “Issued To?” field and select who the citation will be issued to from the list above.

The selection will be used to auto fill an ELCI when the replicate button is selected once the form is validated. ***Generally the inspection form should NOT be moved to an issued status(printed or emailed) until the ELCI (citation) has been created from the replicate function. This will then allow the inspection form to accept the UCT Number and Chk digit number from the ELCI for those fields on the inspection form. ELCI forms created using the replicate button from the inspection form will auto fill all the information from the inspection form except the court information. Once the ElCI has been validated the inspection and ELCI form can be issued. ***

Click on the “Add Next Defect” button to add additional defects.

**All defects created with the “Citation Given? “ field marked “Yes” will open and auto fill separate ELCI (citation) forms once the replicate button is selected from the top menu. Users must remember to complete the “Issued To” field for all information to be imported to each ELCI. The replicate button must only be used once to generate ELCI (citation) forms from a validated inspection form. Any ELCI(Citation)forms created after the initial ELIC replication is done must be done using the Add Form button or opened individually and manually entered.**

Date Completed Field: Enter end date of stop.

Time Completed Field: Enter end time of stop.

Corrective Action & Follow-up Field:

Select “Correct & LEA Sign off” to require all repairs to be corrected within the time given in the “Days to Correct” field and to be verified by law enforcement.

Select “Correct & Mail-in” to require repairs to be made within the time given in the “Days to Correct” field.

Select “No FollowUp Needed” if there is no further contact needed.

Days to Correct field: Enter the total number of days given for the violations to be corrected or repaired.

Enter any additional instructions or notes regarding the traffic stop. This narrative section will print with the form for the violator and cannot be opened for editing once issued. The Officer Notes section will remain on the electronic copy of the inspection report only and can only be viewed by those with access to the form within TraCS. The Officer Notes section will autopopulate to the narrative setion of a citation if a citation is replicated from the inspection report.
