Contact Summary

Contact Summary Form


Contact Summary is used to collect information on a case or stop.

  • Can be used for many things

  • Summary doesn’t contain actual names

  • Can import data from a case into this form

  • Tracks basic demographic information but not names or specific information.


This form doesn’t get transmitted outside your agency, and is used to collect information on a case or stop.

NOTE:

  • The Contact Summary form does not replicate to any form as it is a Case level form and other forms are needed first.

Form Design

The Contact Summary Form contains the following groups:

  • Document

  • Summary

  • Location

  • Unit

  • Individual

  • Law Enforcement Agency

Document Group

Each Contact Summary form contains one Document group. The fields within the group are shown in the figure below.

Summary Group

Each Contact Summary form contains one Summary group. The fields within the group are shown in the figure below.

 

 

Unit Group

Each Contact Summary form can contain multiple Unit groups. The fields within the group are shown in the figure below.

 




Individual Sub-Group

Each Contact Summary form can contain multiple Individual groups. The fields within the group are shown in the figure below.
