A Gallery of Crash Report Examples

Wisconsin law enforcement submits crash reports on an electronic form, the DT4000. These reports are dynamically created according to the crash scenario, and the numbers of vehicles and persons involved. Because there is no such thing as a fixed DT4000 it is not practical to provide a blank crash report. Instead, this gallery will provide a variety of crash reports representing a wide range of crash scenarios. The data provided to create these reports is artificial and is not intended to represent any actual persons or vehicles.

Non-Injury Animal Crash (aka Car Deer)

There is an abbreviated version of the DT4000 that can be used to report single vehicle collisions with a non-domesticated animal that did not result in injury to a person.

 

Fatal Crash

Four-unit (Truck, Passenger Car, Bus, Passenger Car) crash involving Fatality.

Fatal Supplement

The fatal supplement is an additional form provided specifically for NHTSA FARS analysis. It is not incorporated into Wisconsin crash data and not provided with crash report distributions.
