Evidence/Property Form


Evidence is used for managing evidence

  • Not Transmittable: Printable only

  • Restricted: This form requires Access rights to be granted and is not available under the All forms access level. Contains 2 levels of access rights and is not automatically available to everyone in the agency.

    • Evidence Reporter: for anyone that collects evidence. Filling out of form and printing of the bar code and property receipts

    • Evidence Supervisor: for evidence technicians, all ribbon functions to include putting into inventory status and making changes to various parts of the form. Access to all reports

  • See User Guide for using the form

    • Listing of property seized, recovered, stolen, etc. for an incident.

    • Separated into 5 categories: vehicles, drugs, money, firearms, and all others.

    • Linked to the master indexes for vehicles and property.

    • Works with bar code scanners

    • Form locks down after being placed in inventory with only a few areas that can be modified by evidence technicians.

  • Reports available

    • Bar Code – Prints a barcode of the form number that can be printed on regular paper or sticker

    • Property receipt –Report that can include multiple Evidence forms

    • Chain of Custody—2 types

      • Printed when checking out evidence to another agency, contains 5 blank lines to be handwritten chain of custody information

      • Printed when disposing of property showing from collection to disposal.

    • Gun Destruction – Combination report that can include data from multiple evidence forms with a list of firearms being sent to the Milwaukee lab for destruction.

    • State Crime Lab report – Combination report that incudes multiple Evidence forms and one incident form. Incident form includes the state statutes, subjects, victims, officer in charge, location, and illumination information. Evidence forms include the tests to run along with every evidence piece being sent to the lab for testing.

The Evidence form contains following groups:

    • Vehicle Location

    • Vehicle

    • Property Item

  • Chain of Custody

  • Disposition

  • Description

  • Law Enforcement Agency

  • Attachment

How to:

 

Document Group

Each Property form contains one Document group. The fields within the group are shown in the figure below.

 

  • Form/Evidence Number

    • The form number on an evidence form also becomes the

  • Form/Evidence Disposition

    • This is the evidence disposition and once the disposition is entered the form is locked down except for evidence custodians. This is also shown in the forms manager grid in the form disposition column

    • 04-Disposed

    • 41-Temporary Storage

    • 42-Permanent Storage

    • 43-Checked Out

    • 44-Stolen

 

Summary Group

Each Property form contains up to one Summary group. The fields within the group are shown in the figure below.

  • Found in Vehicle

    • This is a form mechanic field and will open a vehicle group attached to the location.

  • Loss Type

    • This is a required field

    • This field is also a linking field to the incident form for property

    • This is a WIBRS populated list.

    • A loss type of Stolen or Recovered will open the recovered type fields.

  • Property Type

    • This is a required field

    • This field is also a linking field to the incident form for property

    • This is a WIBRS populated list.

    • This field also determines which evidence related group will open. There are 5 types

      • Money

      • Drugs

      • Vehicles

      • Firearms

      • All others known as Property Item

  • Submit to Lab

    • If submitting to lab equals yes, then the exams requested and special handling fields will enable.

  • Special Handling

    • If other is chosen in this field the free key Other Special Handling field will open.

  • Recovered Type

    • This field is also a linking field to the incident form for property

    • This is a WIBRS populated list.

    • If Recovered in Other Jurisdiction is chosen the Recovered by agency and ORI fields will enable.

  • Recovered by Agency

    • When choosing one of the options in this field will populate the ORI Field


Vehicle Location Group

This is not a required field and is triggered by a field in the summary group. It merges into the location group above.

Individual Group

There are is one individual group on every form and the user can add up to 10 individuals

  • Type of Person

    • This is a multi select field that can be used for multiple purposes.

    • The current values are

      • OTHER

      • OWNER

      • PROPERTY RECEIVED FROM

      • RETRIEVED STOLEN PROPERTY

      • SEIZED FROM

Evidence Group

This section is broken into 5 different types of groups of which only one can be used.


Drug

If a drug property type is used one group is on the form

  • Loss Type

    • WIBRS populate field that populates from above. Used to help keep track of property in the master index

  • Property Type

    • WIBRS populate field that populates from above. Used to help keep track of property in the master index

  • Drug

    • The Drug list corresponds to the state statutes

  • Quantity

    • Only enabled for Seized Drug loss types

  • Measure

    • Only enabled for Seized Drug loss types

  • Value

    • Enabled when loss type is not seized

  • Recovered Value

    • Only enabled for a loss type of stolen and recovered

  • Drug type

    • Drug field will populate this field

    • This field is also a linking field to the incident form for property

    • This is a WIBRS populated list.

    • Always disabled.


Money

If a money property, then one 1 to 10 money groups are on the form

  • Loss Type

    • WIBRS populate field that populates from above. Used to help keep track of property in the master index

  • Property Type

    • WIBRS populate field that populates from above. Used to help keep track of property in the master index

  • Total Amount

    • Calculated field

  • Quantity Recovered

    • Only enabled for a loss type of stolen and recovered

  • Recovered Amount

    • Calculated Field


Firearm

If property type is firearm than one firearm group will be placed on the form

  • Loss Type

    • WIBRS populate field that populates from above. Used to help keep track of property in the master index

  • Property Type

    • WIBRS populate field that populates from above. Used to help keep track of property in the master index

  • Firearm Type

    • Is populated from the Milwaukee Crime Lab Gun Destruction list.

  • Make

    • Is populated from the Federal list

  • Recovered Value

    • Only enabled for a loss type of stolen and recovered


Vehicle

If property type is a vehicle then one vehicle group will be placed on the form

  • Loss Type

    • WIBRS populate field that populates from above. Used to help keep track of property in the master index

  • Property Type

    • WIBRS populate field that populates from above. Used to help keep track of property in the master index

  • Vehicle Type

    • Is populated from the Crash forms vehicle list.

  • Towed

    • Enables the Vehicle Removed By and Phone number fields

  • Vehicle Removed By

    • Enabled only when Towed equals yes

    • Is an agency populated field that is also found on the crash form

  • Phone Number

    • Enabled only when Towed equals yes

  • Impounded

    • Enables the Impound Location and Impound Tag Number fields

  • Impound Location

    • Enabled only when Impounded equals yes

    • Is an agency populated field

  • Impound Tag Number

    • Enabled only when Impounded equals yes

  • Recovered Value

    • Only enabled for a loss type of stolen and recovered

  • Recovered in parts

    • Only enabled for a loss type of stolen and recovered

 


Property Item

Anything else will cause a property Item group to appear.

  • Loss Type

    • WIBRS populate field that populates from above. Used to help keep track of property in the master index

  • Property Type

    • WIBRS populate field that populates from above. Used to help keep track of property in the master index

  • Additional Property Type

    • Is populated by the crime labs list

  • Quantity Recovered

    • Only enabled for a loss type of stolen and recovered

  • Recovered Value

    • Only enabled for a loss type of stolen and recovered

Chain of Custody Group

One chain of Custody is found on the form. Other custody groups will be added when using the ribbon controls. The last chain of custody group determines the evidence form disposition.

  • Transfer Type

    • This field determines how the form disposition will be updated.

    • This field is populated by the FAC

  • Property Evidence Control Location

    • This field contains both items from the FAC and items from the agency

    • This field is filtered by disposition type

  • Storage Type

    • This is an agency table

    • This field is filtered by the Property Evidence Control Location table

  • Storage Number

    • This is an agency table

    • This field is filtered by the Storage Type table

  • Entered By

    • Determined by the login at the time of adding

  • Badge #

    • Determined by the login at the time of adding

  • Entered Date

    • Determined by the login at the time of adding

  • Entered Time

    • Determined by the login at the time of adding

Disposition Group

There are no disposition groups until the evidence is disposed of. Then there is one disposition group for the form. Can only be entered by ribbon control

  • Transfer Type

    • List Determined by the FAC

      • RETURNED TO RIGHTFUL OWNER

      • CONVERTED TO DEPARTMENT USE

      • SENT TO FINANCE BUREAU FOR PROCESSING

      • FORFEITED FOR DEPARTMENT OR OTHER AGENCY USE

      • PERMANENTLY RETAINED BY COURT/PROSECUTOR

      • DESTROYED

      • PERMANENT TRANSFER TO OTHER AGENCY

      • RETAINED IN CASE FILE

      • PERMANENT TRANSFER TO OTHER CASE

      • OTHER (EXPLAIN BELOW)

  • Date Disposed

    • Date evidence was disposed of

  • Time Disposed

    • Time evidence was disposed of

  • Disposed By

    • Person that disposed of the evidence

  • Badger #

    • Badge number of the officer that disposed of the evidence

  • Witnessed By

    • Witness to the disposal of evidence

  • Witness Badge #

    • Badge number of the witness to the disposal of evidence

  • Evidence Present

    • Was the evidence present at the time of disposal

  • Witness Needed

    • Was a witness needed to the see to the disposal of evidence

  • Notes

    • Any additional notes needed by the agency on the disposal of the evidence

  • Entered By

    • Determined by the login at the time of adding

  • Badge #

    • Determined by the login at the time of adding

  • Entered Date

    • Determined by the login at the time of adding

  • Entered Time

    • Determined by the login at the time of adding

Description Group

There is one description group on every form.
