Print and put the printed copy with the sample to be sent.
Transmit The State Lab of Hygiene will not need to hand enter the data and can use the barcode at the top of the report to pull up the transmitted record.
In Fall 2024 the report is being modified to allow section E. Specimen Collection to be entered on the form. Section E should be entered after collection of sample, and should all or none. This information can be entered after printing blank information so it can be sent via xml transmission to the State Lab of Hygiene |
